Client: I am feeling overwhelmed by the learning, and training task. The amount of information to take in. The skills and practices I have to master. My feelings of inadequacy to the task at hand.


Coach: Please tell me more. Go on, please elaborate. 

[As the client expands and elaborates on his/her learning and training setting/situation, provides background about their prior successes and challenges (with light, gentle questioning), and focuses on what currently overwhelms them, from a physical, mental and emotional standpoint (directed and guided by exploratory questioning), I (as coach) focus intently on what is being said, what words are being used, what specific 'pain points' are being brought up, how it being said, tone, gestures, body language and posture, emotional overtones and undertones, and progressively explore why the client is feeling and experiencing 'being overwhelmed', what 'being overwhelmed' means to the client, and how this is embodied (BEL), before gently directing the clients awareness to observe what and how they are expressing and embodying 'being overwhelmed', and explore actions they might take to obtain different results (OAR).]

Just pay attention and have awareness of your breathing, and posture.

One of the many joys, privileges and deep responsibilities we (i) have as clinicians and healthcare practitioners is to teach, and engage in scholarship (which includes writing, exploring, integrating and disseminating ideas, of which this short reflection is an example). The role of teacher, and educator includes many additional subroles, spanning instructor, trainer, guide, 'coach' (and later as mentor, sounding board, peer-collaborator-colleague-team member or team-mate; and for some of us, for a time, manager, leader and 'coach'). To effectively (and efficiently) perform these roles and responsibilities ideally requires knowledge, skills (ideally acquired through formal instruction and training, including practice with feedback and personal reflection, in an iterative, cumulative, mastery training model; but very often acquired through observation, repetition of what we have been personally exposed to which resonates with us, or 'seems to work'; practices, approaches and behaviours modelled by others - who we admire or 'look up to'); knowledge and skills which ideally should be founded on and build upon a deep foundation of values, professionalism, passion for the science, art and craft of what we do; and personalised to 'why, and what' a student, trainee, colleague, peer, person we have conversations with - work with - teach - and 'coach' really want, and need (and wish for), before either entering a discussion and having a conversation on 'how to' - through teaching, instructing, guiding, mentoring or 'coaching'. Coaching in this context, the coaching role, might be a minor, or a major part of a one-off or longer series of conversations and discussions. Coaching might be performance-technically-feedback-prescriptive focused, or tilted toward capacity building and self-awareness, by building up the ability of the individual being coached to observe themselves in the moment, in action - how and what they think, use language, feel and express emotions, their breathing, tone of voice, and how they use their body, their posture, and posturing - use of gestures. An 'ontological' coaching approach with focus and emphasis on the individual as an aware 'observer', of their body-emotion-language (BEL), and through this observation intentionally (and not reactively, in an unconscious, previous reflex) choose new actions, in order to produce new results (observation-action-results or OAR). My role as a coach, when I put on a coaching hat, and when our conversations tilt toward coaching (away from teaching, instructing, mentoring, guiding) will be to focus on this aspect.

Client: What is the difference between Teaching, Tutoring, Instructing, Training, Mentoring and Coaching?


Coach: You will have experience, both direct and through observation, of being 'taught' and what 'teaching' is in school, higher education, and lifelong learning settings, not to mention during extra-curricular or outside academic activities in school, and recreational pursuits outside study and work; as well as 'tutoring' (small group to personal one-to-one interactive, personalised instruction), 'training' in both large and small group, including (for some) personal settings. You might also have (are likely) to have had some direct experience with 'mentoring' in educational and workplace settings, both formal and informal, as part of 'on-boarding' or orientation process, and at various points in your work (and for some further education and training) journey. 

Elements of 'coaching' occur in teaching, tutoring, instructing, training; and mentoring. However there is a distinct change in emphasis and focus, from technical and task related areas (knowledge and skills), toward 'inner work', with coaching orientated to 'mindsets', 'motivation', 'getting your head in the game' (to use a sporting analogy), and developing an awareness of what you bring to the task - at work, outside work, in and to life. Your body, mind and heart - Thinking, Use of Language, Emotions and How you use your Body e.g. through posture, breathing, tone of voice and speech. The coaching process we will engage in will direct your awareness to first observe yourself (body, emotions, language), the actions you take (and how you might modify these) and results you get.

Thank you for approaching me (directly, via email) for coaching. We will be conducting our coaching session over Zoom, or in person.

Do note that despite, or in-spite of my background experience (below section), the coaching process is all about you, the client. After an informal get-to-know-you conversation, and discussion about the coaching process and a client-coach confidentiality (including coaching ethics and safety) agreement; we will spend our time focused on you, your goals, and concerns. I will spend a major portion of our time intentionally and actively listening to you; intermittently exploring areas you have brought up relevant to you, clarifying and summarising what you have brought up. We will wrap up each coaching conversation planning activities you can undertake before our next coaching session-conversation.

[My technical expertise is in clinical practice (clinical radiology over 30 years, interventional/surgical radiology over 15 years), department administration/operational leadership - clinical radiology department 6 years, patient safety and risk management - 6 years [as inaugural chair, risk management and safety for NUH], professional societies (radiology - College of Radiology, Academy of Medicine Singapore and Radiological Society of Singapore committee member 6 + 6 years, including Honorary Secretary of SRS 3 years; organising committee for local and international Radiology Conferences - including SRS annual scientific meeting, Asian Association of Radiology and International Conference of Radiology including being publications co-ordinator for AAR and ICR and leading daily bulletin team for ICR; 3 International Biomedical Engineering Conferences as member of organising committee, over 15 international medical education conferences as member of organising committee including role as Chair of conference organising committee), medical education [undergraduate, postgraduate radiology] over 25 years, eLearning or Technology Enhanced Learning (TEL) over 20 years, with faculty development in TEL (locally and internationally) over 10 years, scholarship in medical education over 10 years [MHPE from Maastricht University, 2011, and publications see Google Scholar]. Client: Can you describe the coaching process we will be undertaking? What are the main steps or stages?  Which 'model' of coaching will we be using? What are similarities and differences between different coaching models and approaches?


Coach: As we are both from a healthcare background, a clinical encounter analogy might help. There are similarities with a clinical consultation between patient and physician, where the patient seeks medical attention (which may be for physical, psychological/psychiatric, or 'other' ailment, which might present itself in different ways to a patient), and it is the role of the physician to observe, listen (fully, with care, concern, empathy, and insight), intermittently clarifying, exploring and doing deeper dives into presenting 'symptoms' and 'clinical complaints', before if necessary performing a physical examination, ordering tests if necessary, and if neccessary, if advice, reassurance, guidance, 'coaching' is not sufficient, to prescribe 'exercises' (physiotherapy, occupational therapy, physical exercises, lifestyle modifications - which might veer into discussions on nutrition, supplements, sleep hygiene, nature of stress etc.) medications, a surgical procedure; a referral to other colleagues, or therapists; and scheduling follow up visits if necessary, or admission to hospital for further tests etc.

Client: You appear balanced, centered, engaged, alive, happy, and at peace. What is your formula or recipe to reach this state?


Coach: It takes time-effort, practice, experience, learning from - observing others over an extended period of time, personal exploration-experimentation-commitment, deep reading and reflection. This is part of a life-journey. A journey of progressive growth, deepening insight and understanding. Always open, flexible, centered, grounded and resolute. Curious and inquisitive - always a beginner - starting each day and moment with a blank page, an open cup, an open heart-mind-and spirit. Learning and being inspired by other travellers on the path, both the wise, happy and effective ones, and observing those less so. Travelling together, learning from, and assisting each other. One formula or recipe, by no means the only path, or 'the' path, which works for me is paying attention (sufficient, balanced care and effort - commitment) to what I call 'The Five Keys To Life'. These are Health, Time, 'Resources[Inner]-Outer [Supports]-Family-Relationships' (as a metaphor for Inner Resilience-Mindset-Thnking-Experience-Skills-Emtional-Regulation-Adaptability, Outer-Supports-Options, Resources to provide these 'Options', including having a minimum - basic. - sufficient Income, Savings, access to clean and safe Water and Food (on the table) - having 'enough', a 'home' or 'shelter', a place to rest - to feel safe and 'warm'; Family-Relationships and a very few close, 'true' Connections-Friends), Networks-[Roles] (including active participation and engagement with a 'Tribe', a 'Community of Practice' or 'Community of Interest', [where we each-individually-have a 'role', and bring-our-unique-value-add], which can be thought of as an 'Ideal' workplace or practice setting, including for 'passion projects'; Associations, and Platforms - connected and amplified by Trust and Alignment, supported by working and interacting together, both in-person, and digitally-online), and Value-[Impact] (a deep sense of connecting with, and contributing - serving society, giving back, paying forward - to the current and future generations).

Client: Please elaborate on your last statement in our initial conversation, before our first coaching conversation - 'What actions and practices follow on from this coaching session.'?

Coach: Just as I focus on 'deep listening' and 'observation' during our coaching sessions, and we use deliberate 'pauses', during our sessions to give space, and time for both you, and me to reflect upon the coaching process [introduce idea of a blank page, a clean slate, an open space, silence; and later explore the idea of mindful awareness and observation of oneself 'in-action' vs. initial practice of mindful awareness and observation through reflection 'after-action']; I will ask you to engage in the following three 'practices' in between our coaching sessions.

1) Firstly, when you awake each morning, before getting out of bed, and whilst your eyes remain closed, spend a few moments (as short or long as you feel you require), to first feel your body, then observe your breathing, and express happiness, joy and gratitude for the start of another day [where you, even if you are experiencing physical pain (or are pain free), emotional worries (or are care-free), or have pressing tasks and burdens (or are joyfully and happily able to have the option of what you choose to take on, or not, add to your day's journey 'load') or not]. 

You have a blank slate, an unwritten page of your life.

Make some choices, of what you will prioritise, how you choose to feel, move, and think. And do one of these. Immediately. 

Practice this. Every day. [This daily practice is non-negotiable, and part of our coaching agreement, as a regular daily practice you will undertake, including the following two practices below].

2) Secondly, during the day, and evening, intermittently pause, physically-mentally-emotionally (if safe to do so!), and briefly touch base mindfully, and observe where you are, what you are doing, feeling and thinking. Just observe. Lightly. Without attachment.

3) Lastly, at the end of the day, before going to sleep, switch off all electronic devices (for at least 30 minutes before bedtime); connect and spend time, with full attention on your loved ones, your family, and take a precious private moment for yourself [this is not negotiable, at least 5 minutes alone private time with and for yourself], and then write a single reflection on a dairy-journal next to your bed. Read this first thing when you sit up from bed tomorrow morning. This reflection is a record of what you observed during the day, how you listened to your body-mind-heart/emotions, what went well, or not, and a single wish for tomorrow. [This last statement, a single wish for tomorrow is non negotiable and a mandatory 'practice'.] 

As you physically write in your diary journal (analogue please, not digital), feel, really 'feel' and engage with this process, with mindfulness, awareness, and a light heart. Let go of all your worries, concerns, and ambitions immediately after you have written this reflection in your life journal-diary. Physically, emotionally, and in your mind 'let go' of the day, release all your thoughts-emotions-worries-ambitions-physical tensions; and have a dreamless, blissful sleep. Release the day into your journal. Rest and sleep the innocent sleep of childhood.

Client: Will you go beyond just listening, and observing my body-emotion-language, what I bring up and how I do this, how I bring myself to the world or present myself to the world?

Will we explore specific areas of my life - like career, home-life, health (physical, emotional and mental), meaning and purpose?

Will we discuss specific industry specific, and technical aspects of improving my life in healthcare, medicine, nursing, the biomedical sciences; management and leadership?


Coach: We will do all of the above. My main focus as coach will be you, the client. What you bring up. How you bring yourself and present yourself (physically, emotionally and using language) when you discuss areas of your life. This is my coaching hat.

I will dynamically and responsively change hats from the role of a 'general' or 'generic' coach or coaching; to more industry or area specific coaching as you require. As you know I have a background in both clinical practice, as well as education (undergraduate, postgraduate, lifelong learning, and faculty development - training teachers), medical and education research, management and leadership in healthcare, public speaking (including presenting to small groups, large groups, conference settings, and in live media), use of your voice and body (informed from voice coaching, and physical theatre practice), sports (specifically tennis, and the martial arts - we can explore tae kwon do, karate [Shotokan, Kempo], aikido, wing chun, fencing, boxing, muay thai, tai chi; and insights from mindfulness practice, meditation [including TM or transcendental meditation, and Zen meditation], yoga [both physical, mental, and dynamic breathing practices from yoga]. 

We can also explore 'life' coaching, including time management, finding balance, finding and discovering 'your' personal meaning and purpose, including exploring 'Ikigai'; exploring lessons from anthropology [including history of work, and why we work], and ideas from positive psychology.

If you desire, we can also explore the biological and scientific underpinnings of ontological coaching [my clinical background both an educator and learning scientist, and medical graduate + diagnostic neuroradiologist - brain radiologist - focusing on what imaging and biological science, and learning science, including published research in the biological sciences as well as performance and learning science;] and how this informs both the coaching process, and how these insights can specifically apply to you - in your professional and personal life.

Don’t rush to observe the emotion. Why, what generated the emotion? Context? Environment? Inner state?

Thoughts, emotions, feelings or inclinations/actions are informative, of what is ‘missing’ or a ‘need’.

As coach, observe what is said, or unsaid, what is ‘emoted’ (tone, posture), and what is done. Goes for coach or my internal observer too!
